Please note: Cigna and University of Utah health plans are not accepted.

Traditional Medicaid plans are accepted and include Molina, Healthy U, SelectHealth Community Care, Health Choice Utah, and UnitedHealthcare.

Uninsured

  • Selected vaccines, including Hepatitis A & B, Tetanus, MMR, Flu, HPV, and COVID, are available for uninsured clients age 19 and older. Other vaccines may require insurance billing or out-of-pocket payment.

  • Women’s health services are offered on a sliding scale — some may be free if you qualify. Free STI testing may also be available based on eligibility.

  • TCM visitation services require Medicaid coverage. Homebound vaccine services are billed to insurance or provided free for uninsured clients.

Immunizations & Vaccinations

  • Immunizations are one of public health’s greatest successes and a key part of preventive care for all ages. The team is committed to offering affordable vaccines, with free or low-cost options available for those who qualify. Clients with insurance can check here to see accepted plans.

  • Immunization Records

    Click here for instructions on how to access your immunization records. Copies can also be requested in person at any health department location. Please note, there is a small fee for an official copy.

  • Immunization Exemptions

    Parents or guardians may request a medical, religious, or personal exemption from immunizations, as allowed under Utah law (Section 53G-9-303). To obtain an exemption form, the responsible individual must complete the online immunization education module.

Women & Children's Health

  • Summit County’s Reproductive Health and Breast & Cervical Cancer Program offers affordable contraception, family planning, and cancer screenings. Services are confidential and income-based, with some available free for those who qualify.

  • Available contraception options include:

    • Emergency contraception

    • Free condoms

    • Birth control pills

    • NuvaRing

    • Depo-Provera injection

    • Long-acting reversible contraception (LARC), including hormonal and non-hormonal IUDs and arm implants

    Please note: Insurance is not accepted for this service.

  • Free breast and cervical cancer screenings are available for those who qualify, including Pap smears, breast exams, and mammograms. To check eligibility or enroll, call 435-333-1500. These services are also available for a fee if you don’t qualify.

    Eligible women may also qualify for the BeWise program, which offers health screenings and support to improve diet and lifestyle habits, helping prevent or manage heart disease and other chronic conditions.

  • Baby Your Baby is a helpful resource for women during pregnancy and postpartum. For more information or to enroll, visit babyyourbaby.org.

STIs & Communicable Diseases

  • Sexually Transmitted Infections (STIs) are the most reported communicable diseases in Summit County, UT. The Summit County Health Department offers affordable, confidential testing for chlamydia, gonorrhea, syphilis, and HIV. Treatment or referrals are available for those who test positive. Call your nearest location to schedule an appointment.

  • TB testing is often required for employment, immigration, or possible exposure. The Health Department offers multiple testing options and support if results are positive. Skin tests are available at Park City or Kamas on Mondays and Park City or Coalville on Tuesdays. Call your nearest location to schedule an appointment.

  • Rabies risk increases in spring and summer due to Summit County’s bat population. If you think you’ve been exposed, use the Rabies Exposure Assessment Tool to evaluate your risk and receive guidance on next steps.

  • Testing for Hepatitis B, Hepatitis C, and HPV is also available. Contact your nearest location for pricing and to schedule an appointment.

  • Utah law requires certain communicable diseases to be reported to the health department to help prevent transmission and protect the community. A list of reportable diseases is available below. The health department investigates, educates, and intervenes as needed, working closely with the state, local healthcare providers, and other health departments to reduce the impact of disease.

    You might be contacted by one of our staff if you are diagnosed or exposed to one of the diseases listed in the link below. Please cooperate with them and feel free to contact the health department if you would like to verify their identity.

  • Most reportable diseases are automatically reported by labs or physicians. If you’re aware of a case that may not have been reported to the Summit County Health Department, contact the Communicable Disease Coordinator at 435-336-3224 or complete the Confidential Morbidity Report form.

Home Visitation

  • The Health Department offers two programs to assist clients who are unable to visit in person due to certain situations.

  • For clients unable to leave their home due to physical, safety, or mental barriers, the Health Department offers in-home immunizations (including COVID and flu) through a mobile clinic. Eligibility is determined through a screening by a Public Health Nurse. Call 435-333-1531 to learn more or see if you qualify.

  • The TCM Program offers in-home assessments and support for Medicaid-enrolled newborns and their families. Public Health Nurses, Community Health Workers, translators, and Clinical Assistants help identify needs and connect families to local resources. The team also provides education and guidance during this important transition. Visits last about one hour. Call 435-333-1531 to check eligibility.

Certificates

  • Birth certificates can be processed Monday–Thursday before 4:30 p.m.

    • Cost: $22 for the first copy, $10 for each additional copy.

    • In-Person: Call to schedule an appointment at any Summit County Health Department location.

    • Valid ID is required to pick up birth certificates. Additional documentation may be requested to verify entitlement.

    Only immediate family members may request a record and must prove their relationship:

    • Parents listed on the birth certificate

    • Siblings with one common parent

    • Grandparents with a chain of birth certificates

    • Children with the parent named on the requested record

    • Marriage certificates may be required to verify name changes

  • IIn-Person: Call to schedule an appointment at any Summit County Health Department location.

    Valid identification is required to pick up death certificates. View accepted forms of ID.

    Cost: $30 for the first copy, $10 for each additional copy.
